Health Needs Assessment

The Health Needs Assessment provides an overview of the health and wellbeing landscape for the people within the Goulburn Valley Public Health Unit (GVPHU) catchment area, including identified priorities and opportunities for improvement. The information gathered and presented within the document is a culmination of research and consultation undertaken by the GVPHU in early 2023. This document provided the foundations for the Improving the Health of Our Communities Together, Primary Care and Population Health Strategic Plan 2023-2029 (document below).

The document is intended to be used by organisations within the GVPHU catchment for planning, evaluation, presentation and funding applications, among other uses. The GVPHU will look to improve on and expand future iterations of the HNA through further consultation, data analysis and collaboration with partners.

Improving the Health of Our Communities Together, Primary Care and Population Health Plan 2023-2029

Goulburn Valley (GV) Health and the Goulburn Valley Public Health Unit (GVPHU) collaborated with stakeholders to produce the Improving the Health of our Communities Together, Primary Care and Population Health Plan, 2023-2029. This is the first regional Primary Care and Population Health collaborative Plan to be coordinated and facilitated by the GVPHU. It aims to provide the GVPHU catchment, GV Health, its partners and community with clear direction and focus areas to improve the health and wellbeing of our community together over the next six years.

Improving the Health of our Communities Together, 2023-2029 Strategic Community Plan

Having a strong, resilient and healthy community is a shared responsibility, leading to better health outcomes for all. The Goulburn Valley Public Health Unit, in collaboration with our community, has created a Community Version of the above plan to enable access and use by a wider audience. This plan is intended for use by all over the next six years to improve the health of our communities together.
