Requesting Patient Information

For health professionals wanting information for ongoing care purposes, please email hisreception@gvhealth.org.au or phone (03) 4804 4352. Your request must be in writing, on your organisation’s letterhead, or from your business email address. Please provide consent from the patient if you are not the patient’s nominated GP.
